DESIGN A HALLOWEEN COSTUME CHALLENGE!

Prize Winners!
Selection and Notification of the Prize Winners will be on Monday, October 8, 2012.
On Saturday, October 13, the WheatonArts glassblowers will create all winning designs! All winners and their families will be invited to the WheatonArts Glass Studio to witness the prizes being made. Winners will be able to keep their glass ornament! The event is open to the public.
Open to Children Ages 5-17!
TO ENTER: Please design your HALLOWEEN COSTUME on the front of the Entry Form and print your name and information on the back of the form. This is the information that will be used to contact the winners so please make sure it is legible.
You may take the Entry Form home and either mail it or bring it back to WheatonArts (address on Entry Form). *
You may enter as often as you wish but only one (1) of your designs can be a winner.
